Informational packet focusing on your pet's surgery day - includes admittance & discharge procedures, as well as authorization & consent forms. We'd like to make surgery day as easy and stress-free as possible for you and your family.
For your convenience, drop-off appointments are offered. At the time of drop-off, please plan to spend 10 - 15 minutes with our staff to discuss the reason for your visit and patient history. If your time is limited, you may complete this Medical History Form prior to drop-off to help reduce your wait.
